INDIVIDUAL / FAMILY DONATION AGREEMENT & CONSENT

1. Donation Agreement

1. Voluntary Gift. I understand that my contribution to Cheer4Today llc is made voluntarily and without expectation of receiving goods or services of substantial value in return.

2. Use of Funds. Unless I expressly restrict my gift in writing at the time of donation, Cheer4Today may apply my contribution where it is needed most to advance its mission of [brief mission statement].

3. Non‑Refundable. All donations are final and non‑refundable except in cases of proven fraud or processing error.

4. Tax Acknowledgment. Cheer4Today will issue a written acknowledgment for any single contribution of $250 or more. Cheer4Today’s federal EIN is [XX‑XXXXXXX]. Please consult your tax advisor about deductibility.

2. Publicity & Likeness Release

1. Permission Granted. I grant Cheer4Today a worldwide, royalty‑free license to use my name and, if I voluntarily supply one, my likeness (photograph or video) for donor‐recognition purposes in print, on its website, and on social‑media channels.

2. Right to Remain Anonymous. Optional: I may request anonymity by selecting the “Give Anonymously” checkbox on this donation form (or by emailing Cheer4today@hotmail.com); Cheer4Today will honor that request in all public materials published after my request is received.

3. Donor Privacy Notice

1. Information Collected. We collect your name, contact details, and donation amount for transaction processing, acknowledgments, and mission‐related updates.

2. No Sale of Lists. Cheer4Today does not rent, sell, or trade donor information.

3. Data Security. We employ commercially reasonable safeguards to protect your data and comply with applicable privacy laws.

4. Opt‑Out. You may unsubscribe from our emails at any time via the link provided in each message or by contacting us.

4. Electronic Signature Consent

By checking the “I Agree” box below and submitting this form, I consent to the use of my electronic signature in lieu of an original signature on paper. I understand that my electronic signature is legally binding.

5. Governing Law & Venue

This Agreement shall be governed by and construed in accordance with the laws of the State of Virginia, without regard to its conflict‑of‑laws principles.

6. Entire Agreement

This document constitutes the entire agreement between the parties regarding the subject matter herein and supersedes all prior or contemporaneous communications.
